The Affordable Connectivity Program (ACP) is a temporary emergency federal government benefit program that reduces broadband Internet service access for eligible consumers. ACP is separate from Lifeline service customers may continue their existing Lifeline service without enrolling in the ACP Program, may apply ACP and Lifeline benefits to the same service, or may choose to take ACP Program benefits from a service provider other than their existing Lifeline provider. You are only allowed to get one ACP Program benefit per household (not per person). You cannot transfer your ACP program benefit to another person. When ACP ends, your household will be subject to un-discounted rates and general terms and conditions if you continue to receive service. You may obtain ACP-supported broadband service from any participating provider and transfer your Emergency Broadband Benefit to another provider at any time. This form is used for the purpose of verifying your eligibility through National Verifier and enrolling you in ACP benefits with an eligible Public Wireless service plan. This form will not be used for any other purpose.
